What is a Medicare EPC Referral?

A Medicare Enhanced Primary Care (EPC) plan — also called a Chronic Disease Management (CDM) plan — is a GP referral that entitles eligible patients to Medicare rebates on allied health services including physiotherapy.

Under the EPC scheme, eligible patients can access up to 5 Medicare-subsidised physiotherapy sessions per calendar year. The Medicare rebate is applied at the time of your appointment — you pay the gap only.

To be eligible, you must have a chronic medical condition (defined as one that has lasted or is expected to last at least 6 months) and your GP must complete a GP Management Plan (item 721) and Team Care Arrangement (item 723).

Am I Eligible for a Medicare EPC Referral?

Common conditions that qualify for Medicare EPC physiotherapy referrals include:

  • Chronic back pain or neck pain lasting more than 6 months
  • Arthritis — osteoarthritis, rheumatoid arthritis
  • Diabetes with musculoskeletal complications
  • Cardiovascular disease with exercise rehabilitation needs
  • Post-surgical rehabilitation for chronic conditions
  • Neurological conditions requiring ongoing physiotherapy

Speak to your GP about whether a Chronic Disease Management plan is appropriate for your condition. Once your GP issues the referral, simply bring it to your ASPIRE appointment.

How to Use Your Medicare EPC at ASPIRE

1

Get a referral from your GP

Ask your GP about a Chronic Disease Management (CDM) plan and Team Care Arrangement. If eligible, your GP will provide a referral letter specifying the number of sessions and the allied health service required.

2

Book your appointment at ASPIRE

Call (02) 9326 7822 or book online and let us know you have a Medicare EPC referral. We'll note this on your file before your first appointment.

3

Bring your referral to your first session

Bring your GP referral letter to your first ASPIRE appointment. We process the Medicare rebate on the spot via HICAPS — you pay only the gap amount.

4

Receive up to 5 sessions per year

Your EPC plan entitles you to up to 5 Medicare-subsidised physiotherapy sessions per calendar year. Sessions can be spread across the year based on your treatment needs.

FAQ

Frequently asked questions

How many sessions does Medicare EPC cover?
Up to 5 physiotherapy sessions per calendar year under a Chronic Disease Management plan. Your GP specifies the number of sessions on your referral — this may be fewer than 5 depending on your condition.
Do I need a GP referral to see a physio at ASPIRE?
No — you can book directly without a referral. A Medicare EPC referral is only required if you want to claim the Medicare rebate under the Chronic Disease Management scheme.
Can I use my private health fund as well as Medicare EPC?
You cannot claim both Medicare and private health fund rebates for the same session. You choose one or the other per session. Your ASPIRE physiotherapist can advise which is better value for your situation.
Does ASPIRE bulk bill for EPC appointments?
ASPIRE does not bulk bill. We process the Medicare rebate on the spot via HICAPS and you pay the gap. Your GP can advise the expected gap amount when discussing your referral.
