We collect personal and health information necessary to provide physiotherapy and allied health services. This includes your name, contact details, date of birth, health fund details, Medicare information, medical history and clinical notes. Information is collected directly from you at the time of booking and treatment.
Your information is used to provide physiotherapy services, process health fund and Medicare claims, communicate appointment details, maintain clinical records and comply with legal obligations under the Australian Privacy Act 1988 and relevant health information legislation.
We may disclose your health information to other treating practitioners (with your consent), health funds and Medicare for claiming purposes, NDIS or WorkCover where applicable, and as required by law. We do not sell or share your personal information with third parties for marketing purposes.
Your health records are stored securely using Cliniko practice management software, which is hosted on encrypted, Australian-based servers. All staff are trained in privacy obligations and access to records is restricted to treating practitioners only.
